We are looking for an Advanced Social Work Practitioner to join our Hospital Team. This is a key role within the service, combining complex casework with professional leadership, coaching and practice development.

Working within a fast-paced multidisciplinary environment, you will help ensure adults receive timely, strengths-based support that promotes independence, supports recovery and enables safe discharge from hospital.

As an Advanced Social Work Practitioner, you will manage a caseload of highly complex cases while providing professional leadership and expert advice across the service.

You will be recognised as a practice specialist, supporting colleagues to deliver high-quality, evidence-informed interventions and ensuring social work practice remains focused on achieving the best outcomes for residents.

    You will:

    • Manage complex adult social care cases, including safeguarding, mental capacity and risk management issues.
    • Provide expert advice and consultation to social workers, managers and partner agencies.
    • Promote strengths-based practice and person-centred approaches to assessment and intervention.
    • Support safe and timely hospital discharges, working closely with health colleagues and community services.
    • Provide coaching, mentoring and professional supervision to social work staff.
    • Undertake direct observations of practice and contribute to workforce development.
    • Support practice improvement and ensure learning from research and legislation is embedded across the team.
    • Chair professional meetings and contribute to service and partnership initiatives.
    • Prepare complex reports and contribute to legal and statutory processes where required.
    • Build strong relationships with health partners and other agencies to improve outcomes for residents.

    About You

We're looking for an experienced Social Worker who can combine excellent practice skills with leadership, coaching and professional expertise.

    You will have:

    • A recognised Social Work qualification and registration with Social Work England.
    • Significant post-qualifying experience within Adult Social Care.
    • Experience working with adults with complex needs, including older people, people with disabilities or individuals with mental health needs.
    • Excellent knowledge of the Care Act 2014, Mental Capacity Act 2005 and safeguarding legislation.
    • Experience managing complex casework and making sound professional decisions.
    • The ability to provide expert advice, challenge and support to colleagues and partners.
    • Strong communication, assessment and report-writing skills.
    • Experience supporting the development of other practitioners through mentoring, supervision or coaching.
    • A commitment to reflective, evidence-based and strengths-based practice.

A postgraduate qualification in a relevant social work discipline and Practice Educator qualification are desirable. Support may be available for candidates who have not yet had the opportunity to complete Practice Educator training.
